Healthy Families
 
If you do not have health insurance or free (no-share-of-cost) Medi-Cal to cover the medical needs of your children, who are age 0 up to 19, the Healthy Families program provides low-cost health insurance coverage.  Please call the telephone number below if you would like a mail-in application, or call 1(877) 597-4777. You may also pick up an application booklet at many local schools, clinics, service agencies, churches and at all DPSS offices.

What services are offered?
Medical office visits, dental and vision care, necessary medical tests, prescription medicines, some substance abuse services, mental health services and hospitalization.

Who can be covered?
Children ages 0 to 19 who are NOT eligible for no-cost Medi-Cal.

Is there an income limit?
Yes, up to 250% of the Federal Poverty Level (FPL).

To qualify for the program, the family's monthly income must fall between the income limits specified for the child's age and family size as shown on the chart.

Is there a property limit?
No.

Can an individual receive services if he/she has other health insurance?
Yes, but not if the child is receiving employer sponsored health insurance or has received it in the previous three months with some exceptions.

How much does it cost?
Monthly premium of $4 to $9 per child, up to $27 per family and co-payments of $5 for some services. Community Health Plan is the Community Provider Plan (CPP) for Los Angeles County and has the lowest premium.

Is citizenship or legal residency required?
Yes.

How to file an application?
Mail-in applications are available at many local schools, clinics, service agencies, churches and at all DPSS offices throughout the County or call 1 (877) 597-47777 to have an application mailed to you.  For additional information on the Healthy Families program, Click Here.
