Supportive Services
are provided to assist GROW participants to overcome barriers to employment.
These barriers could include substance abuse problems, emotional/mental
disorders or being victims of domestic violence. Treatment programs are
provided for substance abusers. Mental health services include an assessment,
as well as treatment services. Domestic violence services include counseling,
legal services, residential/non-residential treatment, etc.
KEY POINTS
- An evaluation of the need for
substance abuse services is made at intake by eligibility staff. Because
cooperation with substance abuse pre-screening, assessment, and treatment
services is a condition of receiving GR, all substance abuse-related
services are generated/tracked/monitored by eligibility staff.
- If an individual is in substance
abuse treatment, the GROW system will reflect this. Participation
in a substance abuse treatment program counts towards the weekly
participation requirement.
- If the participant requests
substance abuse services or if it is determined that an evaluation
is needed, the participant is to be referred to eligibility staff
for pre-screening.
- The need for domestic violence
and mental health services is evaluated during Orientation/Job
Skills Assessment and
Vocational Assessment. Also, a participant can request
supportive services at any time. This is determined in the GROW
program. The evaluation includes:
- A mental health screening; and
- Self-declaration/request for
domestic violence services.
- Domestic violence and mental health
supportive services are optional. The participant may choose whether
to accept them, when offered. If the participant agrees to accept
these supportive services, they become part of the ABP 1463, Activity
Assignment and, therefore, become mandatory.
- Participation in supportive services
counts towards the weekly participation requirement. Concurrent participation
in supportive services with another activity is limited to 20 hours
per week.
- GROW service
providers can refer participants back to the GROW Case Manager
for supportive services via the ABP 4026, Identification of Participant
with Supportive Services Needs, form.
- Mental Health
- Participants screened as potentially
in need of mental health services are offered referrals for clinical
assessment. The Department of Mental Health (DMH)
conducts the clinical assessments for all offices except Civic
Center and Lancaster. Community and Senior Services (CSS) conducts
the clinical assessments for Civic Center; the Department of
Health
Services (DHS) conducts the clinical assessments for the Lancaster
District.
- The results of the clinical
assessment will be faxed to the Case Manager on the ABP 1466,
Clinical Assessment Provider Referral. If treatment is
recommended:
- DMH's Clinical Assessor
will schedule the appointment with the service provider, indicate
the provider name, location, and appointment date on the ABP
1466, and fax the ABP 1466 to the Case Manager.
- CSS' Clinical Assessor
(for Civic Center) will indicate on the ABP 1466 that treatment
is recommended, returning the ABP 1466 to the Case Manager
to make a referral for treatment to the local DMH office.
- DHS' Clinical Assessor
( Lancaster) will indicate on the ABP 1466 that treatment
is recommended and fax the ABP 1466 to the Case Manager.
The Case Manager will make a referral for treatment
to the
local DMH office.
- Participants have a right
to a third party clinical assessment if they do not agree with
the clinical assessor's results.
- If treatment services
are recommended and accepted by the participant, referrals are
made to treatment service providers.
- Domestic Violence
- Participants requesting domestic
violence services are offered referrals for services, including
counseling, legal services, transportation and other available
services.
- Participants residing in emergency
domestic violence shelters should not be required to attend Orientation,
if it presents a risk to the participant.
- Domestic violence service providers
can make reverse referrals via the ABP
127, GROW Domestic Violence Services Verification form.
- Substance Abuse
- If the participant requests
substance abuse services or if it is determined that an evaluation
is needed, the participant is to be referred to eligibility staff
for pre-screening.
CASE MANAGER DUTIES
- At
Orientation/Job Skills Assessment, evaluate the need for
supportive services:
- Domestic
Violence
- Inform participants about
the availability of supportive services for victims of domestic
violence, giving them an opportunity to self-disclose their
need for these services.
- Mental
Health
- Screen all participants
for potential employment barriers due to an emotional
or mental disorder.
- For participants
screened as needing further evaluation:
- For all districts
(except Civic Center and Lancaster), telephone the
central appointment number (1-213-639-6777) to schedule
an appointment with a DMH Clinical Assessor.
Refer the participant with an ABP 1466.
- For Civic Center
District, telephone 1-213-974-0234 to schedule an
appointment with a CSS Clinical Assessor. Refer the
participant with an ABP 1466.
- For Lancaster District,
telephone 1-661-945-8458 to schedule an appointment
with a DHS Clinical Assessor. Refer the participant
with an AB 1466.
- When the ABP 1466
(from Clinical Assessor) indicates mental health services
are appropriate, refer the participant to treatment
services with an ABP 1467.
- For all districts
except Civic Center and Lancaster: The ABP 1466 from
the Clinical Assessor will indicate the service provider,
location, and appointment date.
- For Civic Center
and Lancaster Districts: The ABP 1466 from DHS' Clinical
Assessor will recommend treatment, but the Case Manager
will contact the local DMH office to schedule an appointment
date.
- During vocational
assessment, the need for mental health and domestic
violence services is evaluated. If the employment plan identifies
any barriers to employment, make appropriate supportive
services referrals, as described above.
- Authorize transportation,
as needed.
- Complete/Update
ABP 821, GROW
Case Manager Checklist.
- If an individual is
in a supportive services activity for less than 20 hours
a week, a concurrent assignment to the next appropriate
activity is made. The individual's weekly participation
in the two activities must equal 20 hours.
- Progress reports are
required every 30 days and will be generated automatically
to the participant by the GROW computer system.
- If the participant drops
out or is not progressing satisfactorily, the GROW system
will automatically put the case into noncompliance
and schedule a predetermination hearing with the Case Manager.
- At the predetermination
hearing:
- If there is good
cause for not progressing, return the participant
to the activity.
- If the participant
dropped out without good cause (and this is the first
instance of noncompliance), return the participant
to the activity.
Notification
from GROW Service Provider
- If notified by a GROW service provider via the ABP 4026, Identification of Participant with Supportive Services Needs form that a participant has a domestic violence, mental health or substance abuse problem, schedule an appointment within ten business days with the participant to discuss supportive services. Determine the need for supportive services and make the appropriate referral, as per existing procedures. Update the GROW computer system and issue transportation and/or ancillary expenses as needed.
- Complete the ABP 4026, Section D ("For DPSS Use Only") and return to the GROW service provider within five business days.
RESULTS
Upon an evaluation that supportive
services are needed, the outcomes should include:
- The participant and Case Manager
signed the ABP 1463, Activity Assignment.
- A referral for mental health assessment/treatment
or domestic violence services was made.
- The GROW computer system was updated
to reflect participants as "shows" for Orientation and a
domestic violence component was opened.
- A domestic violence component was opened,
supportive services were issued and the domestic violence reverse
referral form was completed and returned to the service provider.
- Transportation was authorized.
- The supportive services assignment
screen was completed on the GROW system.
- Participants attending supportive
services for less than 20 hours each week were assigned to a concurrent
activity.
- Upon receipt of the ABP 4026 form, the GROW Case Manager shall ensure that:
- participant's supportive services needs were evaluated;
- appropriate supportive services referrals were made;
- GROW computer system was updated, and
- the ABP 4026 form was completed and returned to the GROW service provider.
